Legal Aid Chicago: Moving Health Forward with Medical-Legal Partnerships

Grantees will learn how Legal Aid Chicago (LAC):

1. Transformed Health Forward from a one-year pilot grant into a successful and sustained multi-year MLP,

2. Leveraged Cook County Health's care coordination network to reach the highest number of patients possible, and

3. Developed a comprehensive care framework to guide health care practitioners in identifying potential civil legal issues.
